1. Support people to make informed choices to promote their wellbeing and recovery, assessing their motivation and capacity for change using appropriate therapeutic interventions
I often discuss with patients about their needs for their session, how much fluid they ideally need to have taken off, how they have been feeling regarding fatigue, Shortness of breath, urine output. Discussing with patients regarding their urine output/fluid input as this can affect their weight. Dialysis can have an impact of the patient mood, they can feel low or become depressed, it is important to be aware of this when interacting with patients as their mood may affect their fluid intake, diabeties management, managing their overall health. Some patients that are awaiting transplant may have been advised by the doctor that they must lose weight in order to be appropriate for transplant surgery. It is important to motivate patients to attempt to lose this weight in a controlled manner.
2. Apply the principles underpinning partnerships in care demonstrating understanding of a person’s capacity in shared assessment, planning, decision- making and goal setting.
Assess a person's capacity to make a decision, for example regarding how much fluid they will aim to have taken off that session. The initial patient assessment is in collaboration with the patient, ensuring we ask the patient a series of questions. Working through the A-E assessment, discussing with the patient to calculate how much fluid theyre aiming to be taken off can be discussed with the patient, we them aim to work in partnership with the patient in order to set a goal for that session of treatment.
7. Maintains accurate, clear and legible documentation of all aspects of care delivery, using digital technologies where required.
I have been using the latops to record the patient data pre and post dialysis, this has been an adjustment for me as I have only ever used paper documentation for recording care given. I find the laptop documentation useful as it enables all healthcare professionals involved in the patients care to see across the patient care received. For example medications, blood tests, 24hour urine, etc. It is also particuarly useful for Dialysis because it enables us as staff to look at previous blood pressures, past fluid taken off etc. To observe any trend s and helps us to assess what is normal for that patient.
8. Makes informed judgements and initiates appropriate evidence based interventions in managing a range of commonly encountered presentations.
