In project management when does an issue occur?
When the tolerances of delegated work have been or will definitely be exceeded

When the project manager is unable to solve it with their level of authority, it is then classed as an issues and escalated to a project sponsor

As soon as the issue is detected and reported to the PM it is logged on the issue register, an analysis is carried out to understand the impact on the project if unresolved, identify impact on scope, time, cost and quality and the business case expected b

PM escalated the results of the issue and risk analysis to the project sponsor with recommended resolutions and mitigation, PS approves recommended actions (in special case will need to be escalated above the PS)

If project tolerances are exceeded and result in scope changes, these are managed using the documented change control process (in PMP). This new plans means a new project baseline which is managed using the documented configuration management process

Risks are potential issues and so depend on an event which may or may not happen, there are not issues until an event breaches the project tolerances

If the PM can deal with the problem within their remit, it should not be escalated to the PS

Barriers to issue management: Failing to further escalate the issue when the issue owner has not resolved it in a timely manner
If an issue is not resolved in an agreed timeframe then that in itself is an issue that poses new risks, so required further escalation

Barriers to issue management: Lack of time or reluctance to identify issues
PM feeling like they don't have time to correctly follow the agreed upon process can lead to serious development issues if easily resolved issues have been ignored or mishandles

Barriers to issue management: Inappropriate tolerance levels
If tolerances are too tight, too many problems will be escalated leading to management resources being misplaced, loose tolerances will result in issues being raised too late for timely corrective action

Supports management by exception, ensures senior management can focus on problems where needed, provides assurance to PS and stakeholders, informs decision making, generated trust and commitment from stakeholders, resolutions can be enacted efficiently
