How are antidepressants stopped?
Gradual reduction in dose over a 4 week period (some patients may require longer periods with drugs with a shorter half-life e.g. paroxetine and venlafaxine. Not required for fluoxetine (long half-life)

What are the key points of antidepressants? (1)
Treat single episodes of depression for 6 – 9 months after remission. Risk of relapse is high and increases with each episode. Chances of staying well are greatly increased by antidepressants.

Reassure patients that antidepressants are: Effective, Not addictive, Don’t cause ‘tolerance’ – they stay effective over time, Don’t cause long term side-effects. There are alternatives (don't stop antidepressants abruptly)

What are the types of extrapyramidal side effects? (2)
Akathisia (restlessness, onset hours-weeks, for treatment reduce dose, switch or use something unlicensed). Tardive dyskinesia (orofacial dyskinesias, choreiform hand movements, onset months-years, treat by stopping anticholinergic, reduce dose or switch)

What is the ideal treatment pathway for schizophrenia?
Antipsychotic 1 -> antipsychotic 2 -> clozapine

Describe features of clozapine (1)
If patient is treatment-resistant. Plasma levels monitored. Aim for 0.35-0.5 mg/L (some patients respond to higher/lower levels). Higher levels (risk of seizures, seizure prophylaxis recommended when levels exceed 0.6 mg/L).

Clozapine levels are affected by CYPP450 inducers and inhibitors. CYP1A2 is the major isoenzyme involved in clozapine metabolism (inducers reduce clozapine levels, inhibitors increase clozapine levels)
